Measuring Process Quality: DPMO, First Pass Yield and Rework
How to quantify process quality with defects per million opportunities, first-pass yield and rework rate — and how a quality scorecard ties them together.
Before you can improve a process you have to measure it consistently. The three standard quality metrics — defects per million opportunities (DPMO), first-pass yield (FPY) and rework rate — measure different things and are frequently confused.
This guide defines each metric, shows the calculation, and explains when each is the right number to report.
Defects per million opportunities (DPMO)
DPMO expresses defect density on a common scale: DPMO = (defects ÷ (units × opportunities per unit)) × 1,000,000. It is the metric behind Six Sigma's sigma scale and lets you compare defect rates across processes with different numbers of opportunities per unit.
Choose opportunities carefully: they are the chances for a defect, not the steps in the process. Counting the same defect type twice, or inflating opportunities, distorts the number.
First-pass yield and rework rate
First-pass yield is the share of units that pass the first time with no rework or repair: FPY = passed-first-time ÷ total. Rework rate is the share of output that must be reworked: rework ÷ total. FPY and rework are two views of the same reality — a low FPY means a lot of rework, which means wasted cost and capacity.

Building a quality scorecard
